What is Semaglutide?

Semaglutide is a synthetic analog of the human glucagon-like peptide-1 (GLP-1) hormone. Endogenous GLP-1 is an incretin hormone released by the L-cells of the intestine in response to food intake. Its primary physiological roles include stimulating glucose-dependent insulin secretion from pancreatic beta-cells, suppressing glucagon release from alpha-cells (which reduces hepatic glucose production), slowing gastric emptying, and promoting satiety in the central nervous system. However, native GLP-1 has a very short half-life of just 1-2 minutes, as it is rapidly degraded by the dipeptidyl peptidase-4 (DPP-4) enzyme.

Semaglutide overcomes this limitation through strategic pharmaceutical engineering. Its structure features a modification at the second position, replacing alanine with aminoisobutyric acid, making it highly resistant to DPP-4 degradation. More significantly, it is acylated with a stearic acid diacid side chain, allowing it to bind strongly to albumin in the bloodstream. This results in a prolonged half-life of approximately one week, enabling once-weekly subcutaneous administration, a groundbreaking achievement for a peptide-based therapy.

The Multifaceted Efficacy of Semaglutide:

1.Glycemic Control in Type 2 Diabetes: In numerous pivotal trials (SUSTAIN program for injectable, PIONEER for oral), semaglutide demonstrated superior hemoglobin A1c (HbA1c) reduction compared to other antidiabetic agents like sitagliptin, empagliflozin, and even insulin glargine. Reductions in HbA1c of 1.5% to 2.0% are commonly observed. Its glucose-dependent mechanism significantly lowers the risk of hypoglycemia when not combined with insulin or sulfonylureas.

2.Weight Management: The effect on body weight has been the most headline-grabbing aspect of semaglutide. At a higher dose (2.4 mg once weekly, marketed as Wegovy®), it acts as a powerful anti-obesity medication. The STEP clinical trial program showed unprecedented results, with participants achieving an average weight loss of 14.9% to 17.4% of their body weight over 68 weeks, alongside significant improvements in cardiovascular risk factors. This is achieved through its action on the brain's appetite centers, leading to reduced hunger and caloric intake.

3.Cardiovascular Benefits: Semaglutide has demonstrated significant cardioprotective effects. The SUSTAIN-6 trial established that, in patients with T2D and high cardiovascular risk, semaglutide reduces the risk of major adverse cardiovascular events (MACE), including non-fatal myocardial infarction and non-fatal stroke. This has solidified its position as a first-line agent for T2D patients with established cardiovascular disease.

4.Other Potential Benefits: Research is ongoing into its effects on other conditions, including Non-Alcoholic Steatohepatitis (NASH), where its ability to promote weight loss and reduce liver inflammation and fat shows considerable promise.

Critical Contraindications: Who Cannot Use Semaglutide?

1.Patients with a Personal or Family History of Medullary Thyroid Carcinoma (MTC): Black box warnings, the FDA's strongest safety alert, are in place based on rodent studies where GLP-1 RAs caused dose-dependent and treatment-duration-dependent thyroid C-cell tumors. While this risk has not been confirmed in humans, the potential severity warrants absolute avoidance in at-risk individuals.

2.Patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2): This genetic syndrome predisposes individuals to MTC, making semaglutide use unacceptable.

3.History of Severe Hypersensitivity: Semaglutide is contraindicated in patients with a prior serious allergic reaction to the drug or any of its excipients.

4.Pregnancy and Breastfeeding: There are insufficient data to assess the drug-associated risk for major birth defects or miscarriage. Its use is not recommended during pregnancy, and women of childbearing potential should use contraception. It is unknown if semaglutide is excreted in human milk.

5.Patients with Severe Gastrointestinal Disease: Given its potent effects of slowing gastric emptying and common side effects of nausea, vomiting, and diarrhea, it should be used with extreme caution, if at all, in patients with gastroparesis, severe gastroesophageal reflux disease (GERD), or inflammatory bowel disease.

Furthermore, it is not indicated for patients with Type 1 Diabetes and should not be used for the acute treatment of diabetic ketoacidosis.
